You should know your rights. Here is some information I would like you to know before we meet:
- confidentiality within the client/counsellor relationship applies to identity of the client as well as the content of the work
- exceptions to this rule of confidentiality will include:
- known or suspected child abuse
- known or suspected neglect of a child
- when a clear and present danger exists to any individual
- a court subpoena requires me to release information to a third party such as a lawyer. In such an event I would notify you of such an obligation whenever possible
- if you require us to release information to a third party such as your doctor or WCB, I would ask you to provide me with written consent to do so.
- all records will be kept in strict confidentiality and will reside in a locked file drawer on the premises
- I will be involved in regular case consultation with peers and/or supervisors. Your case may be discussed, full confidentiality of your identity will be observed.
- aside from your right to confidentiality, you have the right to
- refuse treatment at any time
- access your file
- to be treated with repect at all times
- if you believe your rights are not being respected discuss your
concerns with me. If this does not resolve your concerns
you have the option of filing a complaint with
